Bilateral Indigenous Patient Navigator Grant Program

Apply for funding to recruit and retain navigators who support First Nations, Métis and Inuit patients in the primary health care system.

Overview

The  Bilateral Indigenous Patient Navigator Grant Program provides financial support to First Nations, Métis Nation of Alberta, Metis Settlements and Indigenous serving organizations to recruit and retain navigators to:

  • assist patients overcome barriers accessing primary health care services across multiple service providers
  • support patient advocacy
  • gain access to resources
  • improve care coordination

The program is intended to improve Indigenous patient access and experiences when accessing the primary health care system by:

  • supporting the provision of cultural and community-specific assistance and support to Indigenous persons navigating the health care system or receiving care
  • working to support and promote health literacy and health education
  • organizing and coordinating access to care services and connections to providers within the patient’s circle of care including logistical support (when needed)
  • providing and facilitating individualized planning for Indigenous persons to assist with access to and navigation through the health care system during their health care journey
  • building connections with and between Indigenous patients and service providers – to improve access to primary health care services and overall continuity of care

Funding

Building on the success of our Patient Navigator Grant Program:

  • The Government of Canada has contributed an additional $4 million to launch a second call of Navigator Program funding.
  • This additional funding is for our Bilateral Indigenous Patient Navigator Grant Program.

Successful applicants:

  • will be provided funding for a maximum of $450,000 over 3 years
  • are eligible to receive up to $150,000 per year, inclusive of salary and benefits

Eligibility

The following Indigenous communities and organizations located in Alberta are eligible to apply for the Bilateral Indigenous Patient Navigator Grant Program:

  • First Nations
  • Tribal Councils and Treaty Organizations
  • Metis Settlements General Council and Metis Settlements within the meaning of the Metis Settlements Act
  • Métis Nation of Alberta
  • Aseniwuche Winewak Nation
  • Indigenous not-for-profit organizations, including Friendship Centres
  • Inuit Organizations registered in Alberta

To apply for the Bilateral Indigenous Patient Navigator Grant Program, follow these steps:

  1. Review the Bilateral Indigenous Patient Navigator Grant Program guidebook
  2. Fill out the Program application
  3. Email the completed application to [email protected]

Application deadline

The deadline for applications is February 3, 2025, at 4:30 pm.

After you apply

We will:

  • review all the applications by March 30, 2025
  • notify the successful applicants in writing by April 15, 2025

Reporting

Successful applicants to the Bilateral Indigenous Patient Navigator Grant Program must submit quarterly reports that demonstrate the awarded funds are being spent properly.
