Reporting requirements
These reporting requirements are not related to the Specialized Criminal Justice Navigator grant. That is a separate grant.
Currently funded organizations must submit financial reports and program reports throughout the grant term for community or police-based programs. These reports include:
Quarterly reports
Report due dates
Reports are due on the 15th of each month after each quarter ends:
- January–March: due April 15
- April–June: due July 15
- July–September: due October 15
- October–December: due January 15
Police-based programs
Police-based programs have to fill out both the Quarterly Report form (Part A) and Part B.

Community-based programs
Community-based programs that offer frontline service will have to fill out the Quarterly Report form.

Annual Progress Reports
For multi-year (3 year) grant recipients an Annual Progress Report is required after the first and second year of their grant term. The report must be submitted before the next payment is released, and must include these documents:
- financial statements
- Actuals Financial Report
- annual report/annual general meeting minutes (if applicable)
- updated board member list (if applicable)

Program Evaluation Report
Program Evaluation Reports are due 90 days after the grant term ends and must include these documents:
- financial statements
- Actuals Financial Report
- annual report/annual general meeting minutes (if applicable)
