Overview
A Driver’s Licence History Report (DLHR) is an automated confirmation letter that contains a history of licences issued to you in Alberta from May 20, 2003, to the date and time the report is printed. It includes dates of licence renewals, different licence classes you have held in this time, and your Graduated Driver Licensing (GDL) status and exit date.
The release of information from a Driver’s Licence History Report is regulated by the Access to Motor Vehicle Information Regulation (AMVIR).
You can only get a DLHR for your own driver’s licence. You cannot order a DHLR for someone else.

Information in a DLHR includes:
Personal information
- MVID (Motor Vehicle Identification Number)
- Driver’s licence number
- Full name
- Date of birth
Current or most recent licence information
- Licence issue date
- Licence expiry date
- Most recent operator service
- Licence class
- Graduated Drivers Licence (GDL) status
- GDL exit date
- Condition codes
- Does not include Mandatory Entry Level Training (MELT) endorsement or restrictions related to restricted driver’s licence.
Licence history information
- Issue date
- Licence class
- The driver’s licence service that was processed on the issue date
The DLHR does not show your driver’s licence status or the date first licensed.

How to order within Alberta (in person)
You can order a DLHR from a registry agent.
Step 1. Fill out an application
In the ’Search Product‘ field, write ’Driver’s Licence History Report‘ and the reason why you need it.
To fill in and save this form:
- Click on the PDF link to save it on your computer.
- Launch Adobe Reader.
Open the PDF from within Adobe Reader. You can now fill and save your form.

Step 2. Drop off the application
Bring your application to a registry agent, along with an accepted photo ID. Sign and date the form in front of the registry agent clerk.
How to order if you are outside Alberta
You can request a DLHR while outside Alberta.
Step 1. Fill out an application
Notarized Request for Personal Driving and Motor Vehicle Information
In the ’Search Product‘ field, write ’Driver’s Licence History Report‘ and the reason why you need it.
Do not sign the form until you are at a Notary Public (see Step 2).
Step 2. Go to a Notary Public
A Notary Public needs to verify your identity, and notarize the application form and the copy of your accepted photo ID:
- Alberta or Canadian Driver’s Licence
- Alberta Identification Card
- Citizenship Card
- Immigration Card
- Naturalization Card
- Permanent Resident Card
- Passport
You must sign the form in front of the Notary Public.
Step 3. Send the application
Send the following documents to an Alberta registry agent:
- a signed and notarized application form
- a notarized copy of your accepted photo ID
- any fees required by the registry agent
The DLHR will be sent to you by registered mail or courier at the address shown in the application. Contact the registry agent for specific details.
