Where to apply for AHCIP

Find a registry to apply for or update your AHCIP health card information.

Registries

You can take your Alberta Health Care Insurance Plan (AHCIP) forms to any registry in Alberta, as all registries offer AHCIP services.

When you go to a registry, bring your supporting documents with you.

  • Registries process AHCIP and non-group applications, update AHCIP information (for example, name, address, marital status) and order replacement cards.
  • Processing time can take up to 5 days.
  • Health cards are mailed to your home from the AHCIP office.
  • This is a free service.

You can pick up AHCIP forms at the registry, or download the AHCIP forms and complete them before taking them to a registry. Check your eligibility before applying.

  • Only specially trained health agents can help you.
    • Contact the registry in advance, to ensure they have health agents available to assist you.
  • Registries are not authorized to give you your personal health card, it will be mailed to you by the AHCIP office.
    • Request the registry confirm your mailing address is correct in the AHCIP system.
