Registries
You can take your Alberta Health Care Insurance Plan (AHCIP) forms to any registry in Alberta, as all registries offer AHCIP services.
When you go to a registry, bring your supporting documents with you.
- Registries process AHCIP and non-group applications, update AHCIP information (for example, name, address, marital status) and order replacement cards.
- Processing time can take up to 5 days.
- Health cards are mailed to your home from the AHCIP office.
- This is a free service.
You can pick up AHCIP forms at the registry, or download the AHCIP forms and complete them before taking them to a registry. Check your eligibility before applying.
- Only specially trained health agents can help you.
- Contact the registry in advance, to ensure they have health agents available to assist you.
- Registries are not authorized to give you your personal health card, it will be mailed to you by the AHCIP office.
- Request the registry confirm your mailing address is correct in the AHCIP system.
